Ms. Lo 0 Posted November 22, 2009 Does anyone have Cigna with a 35 BMI? I seem to be having difficulty with them, or rather with gathering all the information needed to get approved for the surgery. Did anyone have to provide records of their weight for the past two years? Also, what type of information did you have to provide (ie journals, records, etc) after completing the 6 month MD supervised dietary program? Thanks!
kristintaylor1 4 Posted November 22, 2009 Hey, Hopefully your surgeons office has worked with Cigna before and should be able to guide you along as to what their looking for. Cigna no longer requires a 2 year weight loss history so not sure if that will helps you. Attached is an updated list of requirements that you have to meet, considering that your plan covers weight loss surgery. www.cigna.com/.../mm_0051_coveragepositioncriteria_bariatric_surgery.pdf
Dana36 0 Posted November 22, 2009 I had to give them a 5 year history of obesity (my BMI was over 40). I used my annual gyno visits for that. I also had to have the psychological clearance, a nutrition class, and a letter from my pcp (again, I used my gyno's office since they are a full clinic who does all of my care).
