LindaD 3 Posted October 2, 2009 I don't think there is any PROOF needed other than the doctor's notes talking about his or her supervision of you during this diet and exercise period. I was (am) Aetna and had to do a 6 month physician supervised diet and exercise program. Each month, my MD sent his office notes to my Surgeon. Julia_N 2 Posted October 2, 2009 I did my pre-op stuff through my surgeon's office. At each monthly visit, they'd ask me how much exercise I was doing, I'd tell them, they'd write it down, and that was that. I was approved with no problem. Good luck! Share this post Link to post Share on other sites
