How is it determined?

I'm not sure if this is the right place to ask this question or if it has already been covered...but any help would be great!

I know that I am close to the 100lb mark, but which chart/ guideline do most doctors use? I find one chart that makes me at the 100 mark and another chart that puts me 10-15 lbs away. How is it determined by the doctors/insurance company as to what is the “ideal” body weight?

My surgons base the ideal body weight on a BMI of 25 any lower is usually unrealistic to reach and most people look too thin at the desired BMI of 19.

Most doctors follow the NIH critera and BMI is a better indicator than your actual weight.
