dolphinz 0 Posted March 5, 2009 Has anybody had this insurance and have to be on a diet for a certain amount of time before they would approve the surgery? I see a lot of people with this insurance have been saying it's been pretty easy, and they didn't say anything about that when I called for the policy information on the Lap-Band procedure. JayTee562 1 Posted March 5, 2009 (edited) Just had to have 6 weighin's in the last 2 years maybe it was 1 year. They didn't have to be in a row just 6 within a time frame. We jsut changed to UHC this year previously Aetna I was needing 6 month weight loss with Dr. My package was sent on 2/6 and approval came back in 3 weeks. carissarenae1986 0 Posted March 5, 2009 Hi, I have UHC choice plus. They did not require for me to be on any sort of diet. My requirements were that I had to have proof of being overweight for 5 years or more, have a BMI of 40 or more, and be on my job for atleast 18 months. I did have a cap though. They would only pay $12000.00, so that only covered the hospital and anesthesia fees. I had to pay my surgeon the additional $5000.00 out of pocket. What really threw me off was that my policy stated that UHC would pay 100 percent of eligible expenses. Yea, they will pay 100 percent up to my cap.
