I need HELP...Please!

I am 32 yrs. old. I have a wonderful husband and two wonderful kids (BOY AND GIRL) and a wonderful Step-son (TEEN).

I am from Newark, NJ. I am going with Montclair Surgical Associates (Dr. Fresco). I have been cleared by the Psychologist, Cardiologist, Gastroenterologist and Pulmonologist...I have 3 more Nutritional visits to go to and I need my PCP to write the Letter Of Medical Necessity with 5 yr. Weight History.

Problem is that my PCPs office has never written one of these letters and I've only been seeing them for 1 yr. I am not a Dr. seeing person and never had a PCP before. Does anyone have a Sample that I may be able to provide my PCPs office with...for them to dummy my letter from. And, do you think it will be an issue that I don't really have much medical history to present? Please reply -- :tongue2:

At my surgeon consult I talked to the lady who submits your paperwork to your insurance and she knew just about everything about every insurance and exactly what you need for everything. You might try asking the same kind of person at your surgeon's clinic for what needs to be included in the letter. Mine provided me with what info needs to be included in each doctor's visit I have to go to (I have to go 6 times during six months of supervised weight loss).

I was concerned too about documentation, but you can use any medical records from anything over the last five years. Any check up where your weight was recorded should be fine.

I did a quick search for you and I found that Bariatric Surgery Has a page that states the proper way to write a letter of neccessity.

Oh, let me just say...I am 5ft 2in. tall and weigh 280lbs. My goal is 150lbs. I can't imagine why I wouldn't get approved---I mean, I work for my insurance carrier...that would really suck if they deny me. I've tried so many things and the only success I had was at 19 yrs.old when I became anorexic for a yr. I was at 200lbs. and lost 50lbs in 4months. Just eating pickles and ice-pops...and on the stationary bike for 2 hours every day. Then I met my now husband and father of my two children who was not having that. So here I am almost 300lbs. and two kids later praying for the opportunity to do it right this time.

Good luck to you! I live near you, but am having my surgery next week at NYU. The documentation depends on your insurance. I have cigna and was shocked that even after the 6 month medically supervised weight loss program, I needed to provide a 2 year history. I called up my many weight loss dr.'s that I had worked with and failed and asked for my records. They had them and they worked!

      Just been waiting until time for my consult with my bariatric surgeon. It's scheduled for Jan 9th. Turns out I won't actually be seeing him. Apparently it'll be with his P.A.             Not sure what to expect. I thought this is where the surgeon would discuss the best surgery option for me. For years I had my heart set on the sleeve, but I've read so many people have issues with reflux - even if they've never had it before - that they've had to be revised to the bypass. I already deal with GERD & take 40 mg of Omeprazole daily, so I started studying about bypass and honestly, it seems like it might be the better choice for me. How can we discuss surgery options if the surgeon is not there?
      What happened at your first consult? Trying to get an idea of what to expect, or maybe I should say, what NOT to expect.
