Letter from Primary care Physican?

Hi everyone, I am scheduled to attend a seminar on Nov. 20 and trying to get the ball rolling a little before I attend. What kind of letter do I need to ask the doctor to write to my insurance company? Also same question for the psychiatric eval? if anyone can help me I would be so appreciative! Thanks, Jennifer

hello Jennifer, it seems like this is the letter that the insurance company wants from your primary just to say that they back or approve of the type of surgery you are trying to get and that you will be a good candidate,they always add the medical terms if you have a condition like Diabetes.:thumbup: The insurance company also wants to know if you have been seen by a primary care for over the years and will want to see that weight and height history(sometimes 5 yrs back). And all the different types of diets you have tried and how much you have lost/gained from each of them. :eek: It's one of the two depending on what the insurance requires for approval. The Psychiatric Eval is something you have to get done for approval also. when i went to my Consult or Orientation with the surgeon he had a list of Dr. to go through for the Eval and once you are done with the (very long) test:cursing: they will either give you a paper of your results or give it to your surgeon to give to the insurance company. I really don't recall but i do know that my surgeon's office collected all my paperwork (i had everything fax to them)and then once they had everything they submit it all to the insuracne company.

I just attended my seminar 2 weeks ago. Like you , I wanted to get as much lined up ahead of time as possible. However, my surgeon hands out a packet of paperwork at the seminar which gets the ball rolling. In fact, I had to sign a form stating I had attended as that is one requirement. Anyway, the packet of information was very helpful. It included the extensive history form, a timeline of what to expect following the seminary regarding calls from their office, their process of filing with insurance to see if your policy covers it, etc.

I'm guessing many programs do something similar to this and will let you know when they need what information. I would love to speed this process up a bit, but am quickly learning I will need a lot of patience (which I am really short on!).

Not sure if this will help, just wanted to let you know that you may receive the answers to a lot of your questions (even those not related to the surgery itself) at your seminar. I think we are all just as curious about the process, etc., as the procedure.
