Pre-approval diet journal

My PCP has told me that I need to make a history of my dieting attempts in a journal style before he can get me a referral to a bariatric surgeon. Has anyone else done this? Do you have an example of one that was accepted? I would like to do this right the 1st time so that I only have to do it once.

Any input is appreciated.

That's kind of strange. When I go to my doctor for my monthly weigh-ins, she just documents what kind of a diet I am presently on. Have you tried Weight Watchers, Jenny Craig, Bariatrics, etc.? If you have tried all of those, try telling your doctor that and give her the dates and what the results were. Here in PA with my insurance I did not need to actually have a referral and I had the option of doing my six months of weight checks at the hospital where the surgery will be done. I chose my family doctor as the surgeon is 2 hours away. I haven't been approved yet, but my surgery isn't until January. Good luck!!

I have to do a 6 month supervised diet also. I found fitday.com. In it I list every bite I eat and all my activities. At the end of the month I go to the overview page and print it out for my doctor. At the appt I tell him I am on a diet similar to south beach.

Sounds like your doctor might want a little more. so maybe you can get a notebook and write down thigs like "ate right today" or "Had a problem resisting crackers" or "emotional day- ate like a pig". You might also jog down your exercise since fitday doesn't have that on it's overview page.

They aren't asking for what I am doing now, they want to know what I have done in the past to try to lose weight. I have tried everything, but I don't remember dates andI didn't keep receipts... I'm just not sure how detailed it has to be and I was wondering if anyone else had to do this and what their experience was.:crying:

My doctor said they wanted details on the past three years of dieting. The paperwork I filled out wanted info on the last three diets. Further back, just use your memory unless you happen to have receipts. Any receipts from any diet program are a real help. I am still waiting for my doctor to send it all in, so I can't tell you how it all worked.

      Day 1 of pre-op liquid diet (3 weeks) and I'm having a hard time already. I feel hungry and just want to eat. I got the protein and supplements recommend by my program and having a hard time getting 1 down. My doctor / nutritionist has me on the following:
      1 protein shake (bariatric advantage chocolate) with 8 oz of fat free milk 1 snack = 1 unjury protein shake (root beer) 1 protein shake (bariatric advantage orange cream) 1 snack = 1 unjury protein bar 1 protein shake (bariatric advantace orange cream or chocolate) 1 snack = 1 unjury protein soup (chicken) 3 servings of sugar free jello and popsicles throughout the day. 64 oz of water (I have flavor packets). Hot tea and coffee with splenda has been approved as well. Does anyone recommend anything for the next 3 weeks?

        All I can tell you is that for me, it got easier after the first week. The hunger pains got less intense and I kind of got used to it and gave up torturing myself by thinking about food. But if you can, get anything tempting out of the house and avoid being around people who are eating. I sent my kids to my parents' house for two weeks so I wouldn't have to prepare meals I couldn't eat. After surgery, the hunger was totally gone.

      I have my final approval from my insurance, only thing holding up things is one last x-ray needed, which I have scheduled for the fourth of next month, which is my birthday.
