2 year diet history?

Hi again -

Did any of you have to do a 2 year diet history for insurance?

I would LOVE to see a sample of one. I realize that I have to do my own, but I really don't know what I'm supposed to list.

I did a 5 year diet history. Just list the types of diets you did, the dates, the amount of weight gained or lost.

My insurance required a 5 year history with monthly check ins with my primary doctor. I just could not imagine waiting another 5 years, so I decided to self pay.

My insurance also required a 2 year supervised diet and after that I would have to pay all of the expenses out of pocket. After 2 years, if I lost weight and kept it off then and only then would they "reimburse" me... It was also for RNY and I am not interested in that type, so it wouldn't have mattered anyway. I too am doing self pay.
