what documentation is needed?

I'm just starting my 6 mon physician supervised diet with my PCP. Exactly what does he need to document with each visit? He is not familiar with LB surgery and says he's not sure of what is needed either.:)

hello your doctor suppose to document , when you go every month to do a weight count.how much you weigh every month you go in. whether you loose or gain.until the 6months are up. then sumit to insurance.

I have been going to my PCP for weigh ins since Aug. I weigh in every time and we talk about weight loss and its documented in my chart however I do not always go just for the weigh in. Is this counted against me? Do I need to schedule appts JUST FOR being weighed in? I hope to have surgery in March or April since I can't start the process until Jan 1st when my new insurance kicks in. Thanks.

My PCP had so many people coming to her for their 6 month supervised diet that she made a template that she used every month to record your weigh and dietary recommendations. I finished my 6 month diet Nov. 7th and did not lose a pound. In fact, I think I gained a few in that period od time. My PCP said the diet wasn't so much to lose weight, as it was to see that you would stick to appointments on a monthly basis. That somehow proves you are really serious about what you are doing. Anyway, All of my paperwork was submitted to my insurance on Nov. 9th and I received my approval letter on Nov. 19th. Good luck to you and God bless.....

My surgeon's office took care of setting me up with a dietitian for six months of visits. I have to track everything that i eat, do meal planning, set monthly goals, learn new eating habits ( the hardest was giving up my daily beer), I weigh in at each visit and we review the previous month's food diary, set new goals, and do some post lapband nutrition counseling. I'm pretty happy with the program but it's very hard to follow a nutritious meal plan. If I could do that I wouldn't need a lap band.

      Just been waiting until time for my consult with my bariatric surgeon. It's scheduled for Jan 9th. Turns out I won't actually be seeing him. Apparently it'll be with his P.A.             Not sure what to expect. I thought this is where the surgeon would discuss the best surgery option for me. For years I had my heart set on the sleeve, but I've read so many people have issues with reflux - even if they've never had it before - that they've had to be revised to the bypass. I already deal with GERD & take 40 mg of Omeprazole daily, so I started studying about bypass and honestly, it seems like it might be the better choice for me. How can we discuss surgery options if the surgeon is not there?
      What happened at your first consult? Trying to get an idea of what to expect, or maybe I should say, what NOT to expect.
