I was trying to find some people who have Anthem BCBS of Colorado (HMO) to give me some advice and let me know things like their requirements, and surgeons. Shinyhappymommy 2 Posted May 24, 2008 I think that is what I have. It's BCBS, (I think Anthem) and my dh is a federal employee. It was great for my surgery coverage. You just need to call them and ask what your bariatric benefits are. Mine covered 90% if my bmi was over 40. It was. If it's 35-40 I would have needed some comorbidities. I have my coverage book right here and all it says it "Bariatric surgery is covered at no more than $7,500 at an approved facility and $1,500 at a non approved facility." So I do understand what is covered, but I'm posting trying to find ideas on contracted surgeon, and find out the approval process.
