Was your plan provided by your employer or do you pay for it out of your own pocket? Employer pays 70% of it, I pay 30% 4.) Are you banded yet, waiting for approval, denied...etc? Was your plan provided by your employer or do you pay for it out of your own pocket? I had to pay for it out of my pocket, because the employer did not have it in the policy. 4.) Yes. It's employer-sponsored, meaning they pay all or part of the cost. In my case they pay part, so I still have premiums taken out of each check. 4.)
