surgeon locator

I went to inamed.com to do a surgeon locator and it won't let me without a username and pw. Okay, so I have to fill out a questionaire. But then I fill it out and they say "you don't qualify based on your entries". ummm, yes I do and I am getting the surgery in 5 days. I just want a list of inamed surgeons in Phoenix Arizona. Can someone who already has a user name and pw look it up and give it to me in a PM?
