Dr. Spivak and Patient Questionnaire

This may be a silly question but since I am going to the seminar tomorrow I want to make sure I have all of my ducks in a row.

The patient questionnaire asks for diet history. How accurate do you have to be? Or are they just interested that you have tried things? I just don't remember all the dates, how much I lost, etc.

i fought my insurance for over 3yrs then ended up as self pay...so in that 3yrs i went to more then several of spivak's seminars and each time had to fill out and update those questionaires.

i agree w/the above poster....its more about everything that you have tried...you can be general on the dates...just guess the month and year...and for however long you tried it.

oh yea, and just a hint, if you are trying to get insurance approval..the more things you have tried the better! HINT HINT!

i would put EVERYTHING on there that you have tried...even the ridiculous diet fads that have come and gone, if you've tried em, put em on there! hope that helps!:wink:

      Just been waiting until time for my consult with my bariatric surgeon. It's scheduled for Jan 9th. Turns out I won't actually be seeing him. Apparently it'll be with his P.A.             Not sure what to expect. I thought this is where the surgeon would discuss the best surgery option for me. For years I had my heart set on the sleeve, but I've read so many people have issues with reflux - even if they've never had it before - that they've had to be revised to the bypass. I already deal with GERD & take 40 mg of Omeprazole daily, so I started studying about bypass and honestly, it seems like it might be the better choice for me. How can we discuss surgery options if the surgeon is not there?
      What happened at your first consult? Trying to get an idea of what to expect, or maybe I should say, what NOT to expect.
