A lot of companies are also making compounded ones that are like $2-300 but it’s not really clear if they are as effective and/or safe quite yet. . My primary provider said they were working on getting the compounded ones in their office so I assume those would be safe but still I question the effectiveness and they are only beginning to work on it now so who knows when it will actually happen. My primary said if it was her she wouldn’t wait for that option to become affordable since my numbers are creeping up.

Anyways, your surgery is getting really close. Are you excited? Nervous? I am already a little of both. Mine is Aug 7. I keep on questioning if I am crazy to go with the newest option just because there isn’t as much help out there for me post op. At least I haven’t found it yet. I already spoke with two nutritionists and both of them claimed to know beforehand about the procedure then both of them pushed the same information on me as they would for a bypass. Total waste of money. I have an appt soon with the NP at my office, though. I’m hoping to get some answers about the regular diet and the supplements that are specific to the SADI.

August 7th isn't too far away! You're close to the preop diet. I started mine a few days ago and it's testing me, but I knew it would. If I were a smaller person, I don't think it would be such an issue. If I weren't having appetite issues even before the diet, things would be easier, too. What's helped has been sipping on broth or slurping on sugar free Jell-O all day. I go slow with those so it feels like I'm eating/taking in more than I am.

I'm only nervous about one thing: the gas pains immediately after surgery. I've never had surgery before, so I am not sure what to expect. I want to have a realistic expectation of pain before experiencing it. I know I will handle it better that way. Luckily, I have a few family members who have had laparoscopic procedures before and we willing to be honest about the experience. I've been avoiding stories online of others' experiences of that moment, because we all have different pain thresholds and it's too easy to go down the rabbit hole of scary stories. Especially with AI/algorithms thinking we want to see the worst of the worst.

I want to share why I made my decision to have the SADI-S vs a sleeve or bypass. While I cannot speak to what things will be like after surgery, I am confident I'm making the correct decision for myself. I'm in my 30s with no kids, but would like some. I have been struggling with my weight ballooning up and down for the last 20 years. At my heaviest, I was over 320 pounds. I hit that as I made the decision for surgery.

I didn't know what surgery I wanted, so I started doing research into the best bariatric surgeons in my state. I read through their websites and looked at reviews for the surgeons through my insurance provider, google, yelp, and other such websites. Then I asked a few friends in the medical field which doctors they would want doing surgery on them. All of that narrowed my list down significantly. I ultimately went on gut instinct and don't regret it at all. My surgeon is amazing. Her teams is extremely supportive.

Here's some information she gave me on my options:

-- Gastric Bypass: She does not recommend the procedure to any patient. It has more points for potential surgery complications and, in her medical opinion, the highest chance for weight regain.

-- Sleeve: potential to lose 70% of excess weight. Less than 1% chance for complications.

-- SIPS/SADI-S: potential to lose 80% of excess weight. Less than 1% chance of surgical complications.

I want to have children, and she advised that the SIPS/SADI-S (there really needs to be a better name for this) is the best choice to allow me to get pregnant a year or so post-op (depending on how I'm doing) and reenter weight loss when appropriate post-birth. I will have to work very closely with my weight loss team throughout the entire pregnancy, but it really feels like a bonus to have more support.

My surgeon's office also recommends patients to very talented specialists for all surgery clearances. They all treated me like they were part of a huge team dedicated to helping me get through surgery clearance. It was amazing.

I was given a packet with all the possible issues I might encounter post-op, what can be done if they happen, and how to avoid them in the first place. When I read through it spelled out in black and white, it's easy to see what my life would need to be to avoid a horrible experience. The most embarrassing ones are noted as being most common with gastric bypass than sleeve or DS. Most of these complications can be avoided by chewing well, not drinking during meals, and not overeating. One of my doctors said I am going back to being a baby again, digestively. I will need to reteach my body how to process what I eat and not be afraid to push back milestones if I'm not ready to start the next leg of the journey.

I'm definitely anxious. The unknown is always scary. I'm confident at the same time because of the team I'm working with. And because of the support I have at home. I know I'm extremely lucky to have the surgeon and support I do. It's definitely a burden to afford this surgery right now, but I don't believe I will feel that way in 5 or 10 years.

Wow. You definitely have done your research and your team sounds like they are amazing. My surgeon is one of the best in my area but he hasn’t done a lot of this particular procedure so his team is definitely not up to date with it. That’s what my appt on the 19th will be about. I am very concerned about my post op care since I will be seeing the PA or NP for follow-ups.

In terms of post op pain I can speak for the sleeve portion. My experience was very little pain. Until I tried to sit up (and felt like I had done about 1000 crunches) I didn’t have any pain at all. I thought that they just put me to sleep And woke me up. I literally worried they didn’t do the surgery for some reason. I hade heard many people with the same surgery who had tremendous pain for a week or longer. I am not sure why but I think more gas gets trapped for some people than others and that’s the majority of the pain or lack thereof. But I was very lucky and even asked them to take me off of pain meds because I didn’t need them. I do have a long torso and kinda wonder if that gave him a little more room to work in there?!

I'm so excited to see activity on this thread! I made the choice to postpone my surgery for a month in order to change from the gastric bypass to the SIPS/SADI-s . (And I agree, there REALLY needs to be a better name for this surgery!) I'm scheduled to have mine on August 5th, so I could definitely go for having folks to talk to who are on the such a similar journey.

Is yours a virgin surgery or a revision??

I'm starting with the SIPS/SADI if my insurance will allow me to do so. I was slated for the Roux-y full Gastric at the beginning of July but after doing a LOT of research I came to the determination suggested above. I spoke with my surgeon, who tried to lightly dissuade me by saying they were both essentially the same results, etc but he was willing to do the SADI, it just pushed things back like I said while they make sure insurance will do their part. I went to my mid-month appointment and all things are moving ahead so far.

Good for you. I hope things continue to go as planned. I have seen quite a bit of research about it as a virgin surgery and it seems extremely promising. Not as much about it as a revision. My surgeon seems to think it’s my best outlook for losing what I want to lose, though, with the added bonus of being able to still take the occasional NSAID.


I am second guessing my decision a little but I think it’s just the typical pre surgery jitters. A month ago when he was ordering tests to verify whether it was appropriate I was afraid that the results would say I couldn’t get the SADI so I know it is really what I want. I’m just getting nervous.

Oh my gosh, me too! But: We got this!!

I'm going to get my lab work done next week and I know it should look really good with as long a path I've been on to get to this point and all the Vitamins I've been taking! I've been taking the Barimelts multi w/iron (dissolved crazy easily, best I've found) the Calcium from BariatricPal every other day, and some Multi-collagen Peptides w/Probiotics for like, two months. I also had to quit smoking cigarettes and with the help of Chantix I've managed to do it, and just passed my fourth month of not smoking! I use the Baritastic app for my food tracking.

There's SO much to the process! I'm nervous and wondering if I'm going to end up feeling like a stranger in my own skin but I'm trying to keep focusing on my reasons for doing all of this - better health, longer life, better quality of life, etc. Feel free to message me; I can be bad about checking my emails but I found an app called Finch that is helping me keep track of everything, thank goodness, and reminds me to CHECK my email. Sorry for the delayed response!

I just got my labs and EKG today. I’m really hoping that’s okay because the lady put four of the little sticky things under one of my boobs. They have always put them on both sides so that was weird. The dr looked though and didn’t say it was bad or anything. I actually just had one like a month ago but they said they wanted the labs to be within a month of the surgery so I said let’s just do it again just in case. I don’t want anything to push my surgery date.

Im trying to get back into the habit of taking my Vitamins too. It’s really hard to remember them. I take the ProCare Health multi and Bariatric Advantage calcium chews but i did try the bariatric pal ones and they aren’t bad. I just want to ask the NP about them before I make that switch. I bought the bariatric advantage chewable multi for the first month.

That’s awesome to hear about the smoking. I quit a while ago and it’s definitely an accomplishment. I remember it being kinda freeing that it was one less thing I always had to keep track of and worry about. And now I don’t now how I ever was a smoker. I smell it and it takes my breath. It’s so weird.

I meet with the NP tomorrow to verify but I think my liver shrink diet is supposed to be a week long. If so, that means next week I will do a low carb diet to prepare. That’s not required but I just don’t want to go from eating like I do now to the LSD. If I’m correct, I will be starting that process this Tuesday.

I am trying to tie up all my loose ends by next week. I don’t want to have anything extra to do the week I am on the liver shrink diet. I figure I will take that time to get my house in order. Every surgery I ever had I sit here and look around at what I could be doing if I wasn’t physically restricted so I hope to get it all done this time.

That's so smart - I have been trying to get my house in order this week, too - this sounds like great motivation to me because I TOTALLY would be doing the same thing; you're right - going crazy about everything I see that needs done at the worst possible time to deal with it!

I hope your EKG is good as well! I'm sure it will be *crosses fingers* I go in on Monday for my Pre-Admission Testing; I'm not sure if they'll do an EKG or not but I'm glad you mentioned it! I was just reviewing my paperwork and it says that "You will have non-fasting labs drawn at this appt. and may have other testing done." so I guess it's possible; I hadn't even thought of it. I'm sure they mentioned it and I lost it in the overwhelming amount of information I've been trying to process.

It really is a LOT to keep track of. I lucked into hearing about that app I mentioned, Finch, from another friend who also had WLS. (Weight Loss Surgery - it took me an embarrassing amount of time to figure out that abbreviation! ) She had the sleeve, and another friend of mine had the Roux-y about a year and a half ago done by the same surgeon I'm using. I've watched her recovery, and it's been rocky. I think doing a 'virgin SADI' as you called it will be way better for me. She's already got an ulcer due to continuing to use NSAIDs and drinking way too much coffee. She also went back to smoking cigarettes. It's been quite the motivator to do this right, I'll say that much!

I start Colace and the liquid diet on 7/29, so we'll be on about the same track! I've got the one week pre-op diet, and then surgery on Aug. 5th as long as I get the insurance and other clearance!

Don’t feel bad about the acronym, I had to ask about LSD (liver shrink diet) just recently and they even spelled it out in the sentence just before it, 🙄.

Ooh yea, NSAIDS that soon after any WLS can really cause issues, even sleeve or SADI. I think I was a year out and had to take them for a vaginal scope thing to look for polyps. I called and they put me in with the NP who said it was okay on rate occasion but I still try to keep it to like once a year at most and even then they say to take it with food and after a PPI.

Please don’t go back to smoking. For the sake of yourself post surgery and just your own sake. I did it. I thought I could smoke just one like an idiot and of course that took me right back like I had never quit. One of the biggest mistakes of my life. It was just as hard to quit the second time. I have no clue what I was thinking.

That’s interesting you start the colace what. The liquid diet. I start right after surgery I think. I should double check. When do you know for sure that they got all your insurance stuff back and your good to go. Seems like they will want to hear back any-day to leave time for someone else to do start their preop diet and fill the slot if they have to postpone you.

Okay so I had my appt with the NP And thank god I asked for it. As I suspected she said she has no clue why they told me I just had to do the two day liquid diet. I thought she was going to say I had to do a week like my sleeve but she said I have to do Two weeks and the two days of liquids. So I have to start Monday. Now I have to cut the rest of my caffeine out even faster and get my groceries for Monday. It’s doable but I don’t like last minute changes. Luckily my LSD isn’t all shakes. I do get a meal and a snack. The meal is 3oz lean mean and 1/2 cup of non starchy veggies with 1/3 cup grains or starchy veggies or fruit. I remember last time I wasn’t starving, just really board with it. I’m guessing two weeks it’s going to be even more boring though. 😳

Thank you! Now I know what LSD means in this regards (Lol) Thank you for the encouragement not to go back to smoking! I definitely still have some psychological dependency on it, even after going the four months without thanks to my Chantix! I notice I start to crave a cigarette when I see someone smoking when I'm late to take it and have to watch myself and prompty redirect and go take a Chantix. I did get my insurance confirmation just a few days ago in the mail and took it with my to my pre-op appointment yesterday; I offered it to them but they said they already had it! (Which is a huge relief!)

Yeah; I was kind of surprised as well that I have to start the colace with my liquid diet, seems redundant and makes me wonder what that week is going to be like, honestly! I'm forseeing a lot of time spent near or in the bathroom, as it stands >. The nurse yesterday did my EKG the same way, under one breast, and they seemed to have trouble getting a good reading because I had to stay SUPER still. They also had to jab me 5 times to get the blood they needed; I was patient and know my veins can be tricky but ngl - I feel like a human pincushion today and have multiple bruises on my arms. I was simply received when they finally managed to get the needle in a vein! They also were telling me to stop taking my Vitamins like, yesterday, and I thought I was supposed to keep up with them until I START the liquid diet, Calling my surgeon's office to get clarification on that today.

I'm certainly nervous and excited about how quickly this is FINALLY coming together! I read that eating lots of jell-o keeps up the illusion of fullness and I'm counting on that and the 'strained cream soup' to keep me from feeling famished. Crossing my fingers I don't turn into an angry hungry hyena this next week for the sake of my family! I'm also a bit uneasy about whether I'll be able to keep enough in my system to keep my meds from making me queasy. Wishing us the best of luck!

Hey, I think I got confused with all of the quoting - @Rosslyn, you're the one who chose to get the 'virgin SADI' right? I'm wondering if this matches what your scheduled surgery looks like - Wish I knew more medical terminology! Anyone feel free to chime in who knows - is this scheduled surgery the SADI or the DS? I'm supposed to be getting the SIPS/SADI-s and this is what it shows on my app:

"I'm wondering about the medical terms I see listed in my healthcare app for what they are calling the surgery and hope I'm getting the one I WANT - also they did tell me I'll need to take an A, D,E & K supplement because of the surgery I chose. Does your scheduled surgery look like this?

I saw the Duodenal and was worried they might be doing the DS as opposed to the SADI - Anyone else who knows, feel free to chime in!

Yeah; I was kind of surprised as well that I have to start the colace with my liquid diet, seems redundant and makes me wonder what that week is going to be like, honestly! I'm forseeing a lot of time spent near or in the bathroom, as it stands >. The nurse yesterday did my EKG the same way, under one breast, and they seemed to have trouble getting a good reading because I had to stay SUPER still. They also had to jab me 5 times to get the blood they needed; I was patient and know my veins can be tricky but ngl - I feel like a human pincushion today and have multiple bruises on my arms. I was simply received when they finally managed to get the needle in a vein! They also were telling me to stop taking my Vitamins like, yesterday, and I thought I was supposed to keep up with them until I START the liquid diet, Calling my surgeon's office to get clarification on that today.

I'm certainly nervous and excited about how quickly this is FINALLY coming together! I read that eating lots of jell-o keeps up the illusion of fullness and I'm counting on that and the 'strained cream soup' to keep me from feeling famished. Crossing my fingers I don't turn into an angry hungry hyena this next week for the sake of my family! I'm also a bit uneasy about whether I'll be able to keep enough in my system to keep my meds from making me queassy. Wishing us the best of luck!

Oh I’m so excited for you to have gotten your authorization. I asked my Dr yesterday about the Colace since I was there and he say I could go ahead and start it too. I don’t remember Constipation being an issue last time but maybe it’s something in the Protein Shakes that can cause it when you have so many in the day? If you mean your bariatric Vitamin I would definitely call. My book says to START them with the LSD. I’m obviously already on them because I had the sleeve but the book is for the general audience who hasn’t been on them for years. 😆

OMG the pincushion thing happens to me too. I wonder if our veins are harder to see because of our weight and that will get better. I have started to actually get a little anxious when I go because it’s always such an ordeal. They usually do a butterfly needle in my hand and then the girl always complains because it just dripping and sticks me again. This time I was like idk how long it takes veins to recover but I have surgery soon and they are gonna need a vein so please let it drip. It will fill up eventually. A friend of mine said they always have a hard time at drs offices and she has much better luck at stand-alone labs. Kinda makes sense since they do nothing but draw blood all day whereas a nurse only does it when a patient needs it.

I assume you are allowed Protein shakes. If so take your meds immediately as you finish one of those. I take my bariatric multi that way and it makes me nauseated without enough in my stomach but with a FULL shake I am fine. I tried to get in a hurry one time and took it when I had only finished half so I could get out the door and it came right back up but I have taken it forever with the full one and it’s fine. The protein in the shakes is actually quite satiating physically. It’s the head hunger that will get you and the Jello does help with that. So do the sugar free popsicles if you are allowed those. It just feels good to sorta chew after a while.

As long as you are conscious of the fact that you may be a little testy because of your diet you should be okay. You could tell your family ahead of time to tell you if they feel like you are not yourself if you think that would help. I am on day two and as long as I keep busy it’s really not THAT bad. Now ask me Again on day 16 and it may be a different story.

      Day 1 of pre-op liquid diet (3 weeks) and I'm having a hard time already. I feel hungry and just want to eat. I got the protein and supplements recommend by my program and having a hard time getting 1 down. My doctor / nutritionist has me on the following:
      1 protein shake (bariatric advantage chocolate) with 8 oz of fat free milk 1 snack = 1 unjury protein shake (root beer) 1 protein shake (bariatric advantage orange cream) 1 snack = 1 unjury protein bar 1 protein shake (bariatric advantace orange cream or chocolate) 1 snack = 1 unjury protein soup (chicken) 3 servings of sugar free jello and popsicles throughout the day. 64 oz of water (I have flavor packets). Hot tea and coffee with splenda has been approved as well. Does anyone recommend anything for the next 3 weeks?

        All I can tell you is that for me, it got easier after the first week. The hunger pains got less intense and I kind of got used to it and gave up torturing myself by thinking about food. But if you can, get anything tempting out of the house and avoid being around people who are eating. I sent my kids to my parents' house for two weeks so I wouldn't have to prepare meals I couldn't eat. After surgery, the hunger was totally gone.

      I have my final approval from my insurance, only thing holding up things is one last x-ray needed, which I have scheduled for the fourth of next month, which is my birthday.
