Is there anything specific I need to include?:nervous All they are looking for is a documented weight for 5 yrs to show you have been overweight. History of being overweight. I called them and asked them for coppies of my chart.... they told me that they charge $ for copies and if my lapband doctor would just send them a fax requesting my chart there would be no fee because it would be a physican to physcian courtesy. So I asked the lapband doctor to do so.... they made me sign a release form saying it was okay for them to request my medical history and they took care of it. I do know other people have had to get their weight from multiple doctor's charts, it does not matter... it is all the same . Oh by the way.... do not get all stressed out on the diet... I actually gained weight and was still approved. :faint: Melissa
