I'm not in this region, but I'm finding it useful. If there are other check lists that are useful please let me know. I'm getting my diet history together to bring into the specialist. blossom 2 Posted February 3, 2005 Get documentation of any/all medical problems. Document any past weightloss attempts whether you did it on your own or with a dr. Document any significant family medical history (heart disease, stroke, diabetes...). Get a letter from your PCP stating that you would benefit from WLS. Remember, your insurance doesn't care why you think you need WLS, they just want the facts, so be sure to give them plenty! Good luck!!! Share this post Link to post Share on other sites
