Anyone had a revision to a Long Limb Bypass?

Hello All,

First Post!

Found this forum very recently, had WLS 10 years and one month ago. Lost 200 pounds but gained back 100 of it. Now I'm 11 days away from having a revision to my RNY to something called a "Long Limb Bypass."

Has anyone here had that done and how did it go for you?

I think it's just where the make the bypass even longer for greater weight loss

Yes indeed. It adds more malabsorption to the mix to help increase the effect. How much "common channel" they leave is always the big question and my surgeon leaves a pretty generous amount from what we discussed. I was hoping to hear from people who had this done to see how it is to live with. Some people (on another forum I used to be on) had me a tad concerned that this is a lot harder to live with than the regular RNY.

Hi there Frosty,

So I had mine done on Nov 11th. I was supposed to have three things done. 1.) Shrinking of the pouch, 2.) Tightening the hole at the bottom of the pouch, 3.) The long limb bypass to something around 200-250cm of common channel. In the end, my surgeon did not change the pouch at all. He said it was an appropriate size and that the restriction I want would come from tightening the hole at the bottom.

I've JUST started eating real food again, so far the restriction is there but it's not the same as when I first had my surgery 10 years ago. I'm also being very careful not to push it, eating primarily mush still so nothing hard or crunchy yet so I'm not sure I'm really feeling it. I also thing my insides are kind of still working themselves out so I'm not really going to be able to say too much just yet but I will say that some of the horror stories I've heard about the frequency of needing to use the bathroom seem to be over exaggerated considerably.

Next week I move to the advanced diet so I may have more to say then and I'll keep updating. On another forum, I was quite put off on the distal or long limb but so far it's been quite good.

Hello @tessarevived , I'm doing quite well. I know everyone's journey will be different but so far mine's going pretty good. Because of the no pouch changes, it doesn't really feel any different. I do get restriction and it seems to come on pretty abruptly but I've managed it fine so far. The weight loss has been slow but steady. I didn't really have a great week as far as diet and exercise goes last week but I still lost 3 pounds.

The only thing which has been a significant change is, and there is no delicate way to put this, but bathroom frequency. I was like a clock before the revision. Once per day, in the morning. Now I'm at least twice and often more than that. I'm not sure if this is just an adjustment or if this is the new normal. Also, without going into too much detail, it's always soft and if I'm not careful of what I eat, it can be accompanied with some kind of liquid or oil. I asked the doctor about it and they said I'm not able to process that much fat in my diet and I should switch to leaner foods. Time will tell.

I am scheduled for a distal on Monday. I had a proximal 15 years ago and it was great! I recently put on all my weight back, and I am so disappointed. I am very nervous about those changes you mentioned. I already have some of those issues, so I am wondering how it can get much worse!!

The way I see it is that you have to take the good with the bad. Ultimately it's all about which is the lesser of two evils. To stay at an unhealthy weight and all the complications that come along with that or to make some lifestyle changes that enable you to work with your revised plumbing. I'm still finding the balance but I've got faith that I will and that in the end, this will be a life saving and life enabling decision. I miss the guy I was 9 years ago and I'm bringing him back!

I know exactly what you mean. I had nearly 15 years with no issues. I miss that girl too. I am nervous as can be. This process has happened so quickly- about 5 weeks! And I haven’t had time to mentally prepare. Surgery is Monday!! I’ve got to get my head in the game.

      Day 1 of pre-op liquid diet (3 weeks) and I'm having a hard time already. I feel hungry and just want to eat. I got the protein and supplements recommend by my program and having a hard time getting 1 down. My doctor / nutritionist has me on the following:
      1 protein shake (bariatric advantage chocolate) with 8 oz of fat free milk 1 snack = 1 unjury protein shake (root beer) 1 protein shake (bariatric advantage orange cream) 1 snack = 1 unjury protein bar 1 protein shake (bariatric advantace orange cream or chocolate) 1 snack = 1 unjury protein soup (chicken) 3 servings of sugar free jello and popsicles throughout the day. 64 oz of water (I have flavor packets). Hot tea and coffee with splenda has been approved as well. Does anyone recommend anything for the next 3 weeks?

        All I can tell you is that for me, it got easier after the first week. The hunger pains got less intense and I kind of got used to it and gave up torturing myself by thinking about food. But if you can, get anything tempting out of the house and avoid being around people who are eating. I sent my kids to my parents' house for two weeks so I wouldn't have to prepare meals I couldn't eat. After surgery, the hunger was totally gone.

      I have my final approval from my insurance, only thing holding up things is one last x-ray needed, which I have scheduled for the fourth of next month, which is my birthday.
