Sleeve revision to RNY or Bypass reason

Today is my sleeve to bypass day. I was sleeved 11/12/2012, and it has been successful I lost 150lbs and kept it off (HW 364, LW 205, MW 216-221). However, the GERD has been horrible!!! Waking up aspirating into my lungs in my sleep feeling like I’m dying because I can’t catch my breath and coughing is horrible. Plus I have a hiatal hernia and developed Hypercontractile Peristalsis (Jackhammer Esophagus), SLE (gained 15 being on prednisone for a year) Sjogrens, and PMR (no one else in my family has had any of this). I love my sleeve never considered the RNY before my sleeve. However, the RNY makes me nervous it’s a nagging feeling I’ve had for awhile. My doctor is regarded as one of the top three in my state. I’ve been discussing this with him for two years. Honestly, my concerns are I don’t want anymore health problems. I guess I dunno I want relief but the feeling is still there!!! Send up some extra prayers for me. Check-in is 10am CST Surgery is at noon CST. Starting weight 235 Preop weight 241 (ate everything I could lol) CW: 227.

This sounds like me. I am way more nervous about the bypass than I was with my sleeve. I had no complications with my sleeve and wondering g if I will be as lucky with rny. I do have an excellent surgeon so that is comforting. I understand exactly what you mean.

I will keep you in my thoughts and prayers today. Please update us when you are able

Why do you want to back out?

I am so afraid that I will have complications with rny. I did so well ( as far as complications go) with my sleeve. Afraid that I won’t be so lucky the second time around.

If I were getting reflux daily I may feel differently. It is bad when I do get it but just don’t have it very often.

So a little bit about me, I was lap banded when I was 17 years old, got it taken out when I was 22 because of pouch enlargement. Now I am thinking of getting sleeve done. But the GERD part scares me, it's like exchanging disease states for one to the other. It helps get rid of Diabetes, but you acquire GERD, so severe to the point that people need revisions to another sleeve or another bariatric procedure. I would be scared to get an RNY or a Bypass, I am happy getting a Sleeve (and this was a decision I thought a lot over to even get to it), so the thought of so many revisions due to GERD scares me to even get this done in the first place. It makes me wonder if I will be one of those who undergoes this too, I don't have current GERD right now, sometimes I do get heartburn maybe three times a month, but I'm not taking anything for it chronically. Anyone have any thoughts? Thank you so much!

So, I’ve read all your activity on the site.

I understand your concerns. Not everyone acquires or automatically has Gerd or needs revision after surgery. Know that many of us have No complications and are doing fine years out. Your surgeon would be the one to talk to about having heartburn three months at a time and considering sleeve.

There are risk and benefits with all types of surgery. Research ad do what’s right for you.

I had my revision on 12/17/19. I too had complications of GERD from my sleeve that I had almost 13 years ago. Since surgery no GERD. I've been on a liquid diet until yesterday and I was able to start Protein Shakes with no problem. Prior to the surgery everything including Water at times would come up.

I lost over one hundred pounds with the sleeve and I did keep it off. I'm currently a bit underweight, but soon I will be able to eat healthier and hopefully live a better life.

I must admit the conversion was a bit more painful, but I'm also a lot older too and my body has been traumatized a lot for the last two years.

I had my surgery at the # 1 hospital in the country. I won't give the name because I don't want to put something they told me without their permission. Prior to the sleeve I had no problems with GERD. The surgeon told me they no longer do the sleeve there because they have had so many patients come to them for revisions after the sleeve due to GERD.

I hope your surgery went as well as mine and you are on the road to recovery.

I am likely revising to RNY from sleeve in May or June 2020 (I would do it right away, but have too many prior commitments between now and May that I can’t). I have constant GERD that has not responded well to any treatments and did have controlled GERD prior to sleeve.

Pretty sure the only revisions from sleeve are either RNY or DS @Dr-Patient

I had a sleeve revision to mini gastric bypass. But it didn't fix my acid reflux and now I need a revision to full gastric bypass because the reflux can't be helped with medication. They say my esophageal sphincter is not closing.

I can only tell you what I am experiencing. I had the lapband......may a thousand curses befall that BS..... Then I got the sleeve. I kinda loved it. I lost 70+ lbs. But now, 2 years in, I have horrific acid reflux. My surgeon said I was maybe 6mo out for cancer. Precipitated by Barrett's esophagus. Now I am sadly submitting to a gastric bypass. I never wanted it. I still don't. But, hey, it beats cancer! Im unhappy, but I am the reason my stomach has been eviscerated.

Day 1 of pre-op liquid diet (3 weeks) and I'm having a hard time already. I feel hungry and just want to eat. I got the protein and supplements recommend by my program and having a hard time getting 1 down. My doctor / nutritionist has me on the following:
1 protein shake (bariatric advantage chocolate) with 8 oz of fat free milk 1 snack = 1 unjury protein shake (root beer) 1 protein shake (bariatric advantage orange cream) 1 snack = 1 unjury protein bar 1 protein shake (bariatric advantace orange cream or chocolate) 1 snack = 1 unjury protein soup (chicken) 3 servings of sugar free jello and popsicles throughout the day. 64 oz of water (I have flavor packets). Hot tea and coffee with splenda has been approved as well. Does anyone recommend anything for the next 3 weeks?

All I can tell you is that for me, it got easier after the first week. The hunger pains got less intense and I kind of got used to it and gave up torturing myself by thinking about food. But if you can, get anything tempting out of the house and avoid being around people who are eating. I sent my kids to my parents' house for two weeks so I wouldn't have to prepare meals I couldn't eat. After surgery, the hunger was totally gone.

I have my final approval from my insurance, only thing holding up things is one last x-ray needed, which I have scheduled for the fourth of next month, which is my birthday.
