You don't like it, its jewellry and its not you AT ALL, you don't think you will ever wear it. Gift giver person said if you don't like it, to tell them (I don't know what the plan was after that - like that they'd exchange it or I'd pick something else etc?). The receipt is (accidently?) still in the gift bag it came in and it was rather expensive. Do you tell them you don't like it??? And if so, how do you approach that? Thats a hypothetical, of course. IF you know them well, I mean very well thank them and tell them it is not really you. Ask them if they would be ok if you exchange. If you don't like them do it anyways and apoligize later...lol..
