I had to call and make sure that what i got was right, but they double checked and yes, it's true! I'm scheduled for May 7th. Some require documented issues with photographic evidence, some just take the doctors recommendation. What is required is defined by the insurance company. They often work together). I had taken pictures of rashes for six months, but did not need them. Plastic surgeon took his own pictures of the skin and that was it.
