History of weight loss

My insurance requires a history of failed attempts at weight loss and I'm a bit worried. I don't have any records of trying to lose weight as I've always done it on my own instead of doing weight loss programs like Weight Watchers. What exactly are they looking for? Would something like a history of recorded weight from myfitnesspal be good enough? I took a weight loss pill, Phentermine, years ago but not sure how I would prove that now. This is the only requirement I'm worried about.

Mine was fine with the surgeon confirming that you'd tried before. But he's the kind of guy who knows this is a last resort, so it was like "so what have you tried before" type on conversation. Ask your surgeon what usually happens for his patients, but it wasn't a problem with mine.

Hopefully it works out this way for me but I'm putting all I can together just in case. I found the prescription my doctor gave me for the Phentermine and I have my weight log going back to 2013 that clearly shows my struggle with weight loss. Hoping this is enough if I need actual proof.

This is where I am, my insurance requires failure of a 180 days of a medically supervised plan. I was told it meant monthly weight checks with a Dr and notes on what I was doing to lose weight.
You should be able to get your medical records to show when you were prescribed the meds, I know I have that in my file too. Good luck with your journey !

Unfortunately I was stuck in the, "I can do this on my own " ,"I'm not THAT bad" or " what will people think?" So I never did anything publicly. The closest I ever did was my fitness pal but I would get frustrated and not stick with it for a long enough time. But my last Dr appointment and hearing that a friends brother, who was only a couple years older than me just died of a massive heart attack due to his weight...45 days after getting married :( this was a huge wake up call and I need to do something for me and my family.

My insurance company just took my word for it. But they require medical records from last 3 years showing I was over weight. So it was a combination of those 2 things that proved my efforts.

      Just been waiting until time for my consult with my bariatric surgeon. It's scheduled for Jan 9th. Turns out I won't actually be seeing him. Apparently it'll be with his P.A.             Not sure what to expect. I thought this is where the surgeon would discuss the best surgery option for me. For years I had my heart set on the sleeve, but I've read so many people have issues with reflux - even if they've never had it before - that they've had to be revised to the bypass. I already deal with GERD & take 40 mg of Omeprazole daily, so I started studying about bypass and honestly, it seems like it might be the better choice for me. How can we discuss surgery options if the surgeon is not there?
      What happened at your first consult? Trying to get an idea of what to expect, or maybe I should say, what NOT to expect.
