Today i feel really nervous, I didn't told at work that I have the surgery. My plan is to say that this night i start with a huge stomach pain and the I had a surgery but I really feel guilty, irresponsible and ashamed

You are taking responsibility for your weight by having the surgery. You should be proud of yourself for making this decision. Whether you tell your co-workers or not is up to you.
