Surgery Approval with Cigna

Hi, I've been given the run around with my insurance company, part of it is part of my surgeons office fault. I was denied the first time because we failed to provide all the documents such as my last supervised visit and recommendation from my pcp, well we've now have all that squared away and I'm currently waiting for a approval. My question is has any one ever been denied with Cigna for not having any conditions such as sleep apnea, high blood pressure etc. oh and btw my BMI 40+

im waiting for approval from Cigna too.. I don't have sleep apnea either... My BMI is over 40 too... no major health issues but high blood pressure and heart disease runs in my family on both sides
