I live in NY, can anyone recommend a place anywhere in NYC, Queens or Long Island where I can go do the psychiatric eval?

Do you have insurance? I just went on my insurer's website and found a psychologist with a nearby office who does bariatric screening.

Ones to see. Insurance should pay for it or portion of it.
