BCBS did the same thing to me. Luckily my surgery date is still two or three months out. it ended up that they made a mistake the letter "just gets printed" so I contacted the insurance company. after several hours of research to figure who to call. There is an insurance coordinator at the facility that you were going to get the surgery done at contact that person and they will tell you who to contact at Blue Cross Blue Shield. BCBS finally said that it was an approved facility

I had to jump through many hoops with BCBS. I'm glad you got it worked out. I almost didn't get my surgery because my height on my original med records did not match my Bari doctors records so the BMI was wrong. It was a mess
