I have a bmi of 47.8 along with bipolar. Also on united and getting banded 10-19-16. Insurance approved.

I have UHC and I am getting sleeved in Dec. With my policy, a BMI of 40 or higher qualifies you for surgery without the need of a co morbidity. They also required a 5 year weight history, a 6 month medically supervised diet and a psych eval. I had to go through all of the doctor's pre-testing as well.

I have United Health Care and I was approved 2 weeks after first doctors appointment on 8/25/16. I was banded 10/13/16.
