Anyone have any experience with using Premier Healths insurance??? I was just wondering how I could look up their requirements for the sleeve to be covered

I have Premier. I had to have 6 months of NUT visits, psych eval and PCP clearance. It took weeks to get my approval letter, which drove me crazy. My date is Oct, 3rd. The process seems like it took forever.
