Bcbs federal bmi question

I'm confused... I have BCBS Federal Basic. One of the requirements is that you have a bmi of 35-39 with comorbity or 40+, u also have to have a history of obesity for the last 2 years. My question is does the bmi have to be over 40 those last 2 years or at least 35. In 2014 and 2015 I have bmi at 35 and 36.

It really isn't cut and dry. They are going to look over many things. I have Federal BCBS also. I provided the following and was approved within a day of them receiving the request.

1. BMI over 40
  • provided PCP Weights doctor notes showing numerous in office weigh loss program with my BMI over 40
2. BMI over 35 with co-morbitalities
  • sleep Apnea diagnoses provided
3. Three year history of high BMI
  • provided PCP notes showing numerous in office visits with BMI 37 - 40 BMI
  • on a separate attachment provided 7 year weight history, I have a history of all my PCP weights, up and down, up and down, since 2008. On my PCP letterhead he gave to me.notated dates
4. Three consecutive month diet history within the year leading up to the request

5. Psych Evaluation
6. Nutrition Class

      Just been waiting until time for my consult with my bariatric surgeon. It's scheduled for Jan 9th. Turns out I won't actually be seeing him. Apparently it'll be with his P.A.             Not sure what to expect. I thought this is where the surgeon would discuss the best surgery option for me. For years I had my heart set on the sleeve, but I've read so many people have issues with reflux - even if they've never had it before - that they've had to be revised to the bypass. I already deal with GERD & take 40 mg of Omeprazole daily, so I started studying about bypass and honestly, it seems like it might be the better choice for me. How can we discuss surgery options if the surgeon is not there?
      What happened at your first consult? Trying to get an idea of what to expect, or maybe I should say, what NOT to expect.
