gina171 626 Posted June 25, 2016 (edited) You really need to call your insurance company and ask, as it is rarely a single answer. Different employers can put in additional requirements, or remove requirements, or they can completely remove bariatric surgery coverage. There can be many different answers to a plan with the same name. Sorry that there's no solid answer. Give them a call, they usually are open on weekends. Ginger Belle 54 Posted June 26, 2016 I know it might differ but for me I have Aetna Choice Comprehensive PPO. I had to do a 3 month disciplinary program (make sure it spans out for 90 days) with a end of program visit with my doc to discuss my progress on the program. I had to do a psych eval, upper endoscopy and a stress test. Thats basically it for me. I was scheduled for the 11th of July but had to postpone because my 3 month program didn't span out over 90 days so I need one more appointment for this month which is scheduled for the 30th. End of program appointment with doc hopefully around July 6th. Everything would be submitted to insurance again with hopefully a new surgery date for end of July or early August.
