Caribear 184 Posted June 24, 2016 (edited) My insurance requires that my doctor supervised diet involves the doctor counseling me on exercise and diet, specifically. I would call them and tell them that you are planning weight loss surgery, and that you need them to help you with the supervised diet. It is also a good idea to get your insurance company to send you a written copy of your coverage for bariatric surgery, if possible. AmberMc03 59 Posted June 24, 2016 My 3 months of dr supervised visits were the initial visit and weigh in, then 3 more appointments with the surgeon where I'd weigh and then talk about what I was doing to work toward the goal. My insurance didn't require a specific amount of weight loss just had to show loss each time. Once I had my last weigh in my file was submitted to insurance for approval.
