My insurance requires that my doctor supervised diet involves the doctor counseling me on exercise and diet, specifically. I would call them and tell them that you are planning weight loss surgery, and that you need them to help you with the supervised diet. It is also a good idea to get your insurance company to send you a written copy of your coverage for bariatric surgery, if possible. Some companies are picky about what your visits entail, some not so much.

My 3 months of dr supervised visits were the initial visit and weigh in, then 3 more appointments with the surgeon where I'd weigh and then talk about what I was doing to work toward the goal. My insurance didn't require a specific amount of weight loss just had to show loss each time. Once I had my last weigh in my file was submitted to insurance for approval.

I was told that I needed to schedule 3 appointments with my family Dr to discuss my weight? So I just call and ask for an appointment to discuss losing weight each time?

Be sure you know exactly what is required first and make sure Dr is documenting what needs to be at each visit. My visits were with my Bariatric Team (Nutritionist).
