I have been looking into the surgery for a while, but I am going to be self-pay so the cost has been very prohibitive for me. But I have gotten to a point that my health is worth more than money. Of course I can't give you any recommendations on that. But I can't suggest doing as much internet searches you can find people who have done it get their opinion. Contact NIH. Who knows they may have a study and can help you out. Well worth investigating. And if I can help in any way please don't hesitate to ask.
