KeepCalm 223 Posted May 19, 2016 They said that it wasn't proven to be medically necesary - I think it was my doctor did not submit Everything they needed - they changed insurance cordinators since then and I got them a log of my gym check ins - because I had to have 6 months of dietary/supervised exercise program, AND show a weight loss... but guess what...
