Read2016 658 Posted May 16, 2016 (edited) I suggest call your insurance and ask them. Get your case number from the insurance so you will have that for the next office. If they say ok, call the new doctor your considering to see what they say. The approval is usually for the procedure not the specific doctor and it usually last for a certain amount of time.
