EKG

STRESS TEST

1 PULMONARY VISIT

UPPER GI

3 PSY VISITS

I WAS VERY DEDICATED WITH GETTING EVERYTHING SUBMITTED TO INSURANCE ON TIME.

I was submitted on 2/4and got my approval 2/8
