rdharri 46 Posted September 18, 2016 Trickle prime approved me for Oct.4th. I'm suppose to have to pay up to my deductible. I'll let you know how much out of pocket as soon as I know. Also how do you provide your documentation for your unsuccessful weight loss attempts I had kaiser before Tricare do I just send them my Medical record from Kaiser? flip26 10 Posted September 19, 2016 @Smdnght I am standard too, I picked my surgeon first and called the office and asked if there was a different referral form for my pcm to fill out. I brought that form with me to my pcm appointment and had her fill it out. That form got turned in with all my records including my doctor notes of unsuccessful attempts at weightloss. sarcruze 24 Posted September 21, 2016 (edited) @@flip26 Congratulations on your surgery. I hope u r feeling well. @@Smdnght28 : It all depends on your Surgeon's Bariatric program. I had to see my PCM and get a referral approved by Tricare and sent to my Surgeon's office stating that they recommend me for the Bariatric program. Once they got my referral they called me to schedule an appt to see the surgeon and if he thought I was a good candidate I immediately saw the patient advocate and set up other appts and she went through the paperwork and a quick run-down of what the program is about and what to expect. It's all gone pretty fast for me. I never had failed diet attempts that were monitored by my PCM so I have to do 4 months of weigh-ins and that is per my surgeons program.
