Pre op weight loss

Does insurance expect you to lose weight in your supervised 6 month diet? I know you can't gain weight, but what if you only lose a few pounds in those 6 months? Anyone been denied for this?

I had a 3 month diet, was trying to quit smoking, AND had a foot surgery, so I'd only lost 5 pounds. Insurance approved me within 48 hours I had Cigna.

My insurance required the 6 supervised diet. Originaly I did the requirement with my primary doctor and didn't lose any weight. Then I started working with a weight losss program. My best friends insurance only required her to meet with them for three months and she couldn't gain any weight.

You have to just be at or below your initial weight. You don't have to lose but you can't gain.
