Dprincess 0 Posted October 30, 2015 Hello, In 2016 I'm will be on my husbands insurance and it covers up to 80% for Bariatric Surgery. Does BCBS Anthem PPO require you to do the 6 -12 month supervised weight loss? I'm trying to get a primary doctor now to start if that is the case....
