xylena88 6 Posted October 20, 2015 My insurance requires 6 months doctor monitored diet, so I have been on for the last 4 months. Dec 2nd is my 6th app, so then it goes to my insurance for approval. Once approved I go on the 2 week diet before surgery. My health care advocate with my insurance ( who will review for pre authorization) is hopeful it will be approved quickly as long as nothing is missing.
