tssiemer 85 Posted August 26, 2015 I was just called and scheduled my consult appointment for Sept -10th and my psych eval for October 7th. She said all my insurance requires is a form proving I've tried to lose weight for 6 months on my own . How do prove this? lsubabe33 23 Posted August 26, 2015 You have to go to your doctor for 6 months. They document everything from how much weight u gain or lose each time. Plus, they give you diet and exercise plans. I'm speaking from what I just had to go thru to get approved for insurance. It was a long list!! Fortunately, had been going to dr on a monthly basis complaining about my weight. jane13 2,256 Posted August 26, 2015 @@tssiemer - you can prove your weight loss efforts because WW has attendance records and if you have a gym membership and can prove you took either some classes (spinning, zumba, Water aerobics) you can probably use that as well, I did something very similar.
