Just got my packet from the hospital! There's is so many things I need to do and fill out! I'm just so overwhelmed and feel like if I make one mistake on the packet I will be denied because I'm barely matching all the criteria they need for the WLS

HELP!!!

I've been on a medical weight loss through my PCP

I'm 205, 5'3 with only one comorbity
