I have Banner health insurance and it took them about 7 business days but they said it could take up to 17 business days

I have Cigna. I was told they have 30 days to approve or deny, however mine was approved in 3 days.

Typically your surgeon's office will call you. In my case, my insurance called me before my surgeon's office had the chance. You can always call your insurance a couple of days after your paperwork has been submitted.
