Yep, and had to tell it to many different people many different times. I didn't have to be very specific...as in giving dates or anything....but I had to say that I had tried Weight Watchers, Jenny Craig, Southbeach.....yada yada yada....

I had to have some months logged with my PCP for a supervised diet...no certain number of months. And no BMI history. (United Healthcare PPO)
