tamg26k 479 Posted March 6, 2015 My eval took about an hour. I had to fill out a questionaire with no less than 200 random questions. I think the actual test was to see if I would finish or give up. Hoven 33 Posted March 7, 2015 Mine took about 30 minutes. I was asked a bunch of questions. Then I filled out a 25 question form and that was followed up with some additional questions and then he printed my letter and I was out of there. They noted that if the 25 question form did score right that I'd have to do a much longer one. I was given a list of about 20 people that could do the evaluation, but they noted one took my insurance and that was who I went to, so I paid a $30copay. joatsaint 2,814 Posted March 7, 2015 Mine was several hours, because there were several hundred questions. Then I had a 20 minute interview with the psychologist. Mainly I was asked questions about how I looked at life, was I prepared for the changes I'd need to make and if I had realistic expectations of how much weight I could expect to lose. Angelpoet73 8 Posted March 8, 2015 I had a $60 copay, 344 questionnaire, a few other forms and about 10-15 minute visit with the doctor. I did all of this in about an hour.
