catherinecaddle 1 Posted February 7, 2015 I live in Fort Lauderdale Florida and Im having a huge problem getting a letter of medical necessity writtem correctly from my Primary care Doctor. I really dont understand him because he was the one that referred me to a Bariatric Doctor for wls. I've done everything that is required of me to get the sleeve. I'm in need of a pcp in Fort Lauderdale or near that accepts Humana so that I can get a letter of medical necessity so that I can have my procedure done. BLERDgirl 6,417 Posted February 7, 2015 @@catherinecaddle - Have you met with a bariatric sugeon yet? My PCP wasn't even involved in my insurance request. My bariatric doctor submitted the request. He was able to write the letter of medical necessity. My Primary only found out when the insurance company CC'd her a copy of the authorization. Share this post Link to post Share on other sites

JerseyCityGal 231 Posted February 8, 2015 Bring the instructions that detail exactly what points need to be made to his office. I think there were 5 points that my nutritionists letter had to cover, so I bulleted them to make sure they were made. Same thing with my psychiatric letter.
