I need a form please!

I talked with my insurance company and I need to document 3 months of my eating, basically everything I eat every day for three months. Does anyone have a sample form or sample month of meals I can use?

Just go online, find a monthly calendar, print out the months you need. Then research high protein/low carb diets. :) pretty easy :)

You can find a free food logging website that has an easy print option. My Fitness Pal is one, and another is Fitday. Good luck!
