Ready set go! 20 Posted December 23, 2014 So I have Illinois medicaid and am in the process of my 6 month physician supervised diet. I don't feel like my doctor is documenting my attempts at weight loss very well. She more or less writes weight loss of how ever much and then continue exercise and Portion Control. That's it. teacherlady2133 258 Posted December 23, 2014 I have Ohio Medicaid and had my Dr document medical diet too. The key was to see them every 30 days for weight loss recorded and notes that you talked about weight loss or diet..that can included food choices and portion control.. The notes were not really detailed... Weight recorded why I was there weight loss and sometimes it had notes about the topic... This help count toward the 6 months I had 3 with my Dr. And 3 with Nutrition from program. Good luck Share this post Link to post Share on other sites
iceciley 6 Posted December 24, 2014 Keep in mind that these physicians work with the insurance companies daily and know what to submit to be approved, they don't get paid if you don't have the surgery.
