wisconsin title 19 requirements?

Hello, I am in the process of started my visits and i am wanting to know what is deemed medically necesssary per title 19? I called the customer care and I was informed only my provider can request that information? If anyone can help me I would appreciate it??

That would be if you have hight blood pressure, if you are diabetic, sleep apnea...that would deem you medically necessary. I hope that helps.

Do I have to have all the conditions? And is high cholesterol (hyperlipidemia) are u in Wisconsin?

My BMI was 39 so I needed two more conditions. I have high blood pressure and acids reflux. So if you have a BMI of 40 or over you should be good to go. I am almost sure. Everyone case is different.

I hope to have my surgery by late Oct. / early November. I am waiting on my last dr. To send in his report. The visit was 9-16 ridiculous, it's almost 3 weeks
