JamieLogical 8,713 Posted October 3, 2014 (edited) I emailed my FMLA paperwork to my surgeon's office (Obesity Control Center) and they filled them out, scanned them in, and emailed them back to me, which I forwarded on to my HR department. bulldog1973 88 Posted October 17, 2014 I guess I am lucky with my PCP. He has been very supportive of my decision to have this surgery since the beginning.
