If your insurance has an exclusion does that mean they won't even consider it even with a letter from the doctor and proof of previous weight loss attempts?

My best advice is to call your insurance company directly. They all are different, and they will be able to explain the exclusion fully.

An exclusion means it is not. Clouded, period. If you are denied but it is in your policy then, yes, you can try more documentation.
