Approved removal and revision...2 letters or one

Okay...what a long process. First request in December for both band removal and bypass revision surgery....

Band Removal - approved

Bypass revision - denied

more stuff to do, etc, peer to peer review .....now this done April - Bypass RNY -APPROVED!! Don't get too excited yet....now my surgeon's office said they have to have both approvals on ONE letter. They cannot use the two separate approval letters??? What?? So, now I am waiting for the insurance company to send them a new letter w/both approvals. They have been trying to get thru to the insurance since Monday this week, no go yet. I am sooooooo getting worn out. Anyone else hear of this before?
